To file a new complaint, please enter in the box a unique title that succinctly describes the nature of the complaint. A drop down list of previous titles will appear, you can select one of them to view or edit that particular complaint. Continue entering a unique title to file a new complaint.
Once you are done entering the title (either for a new or a past complaint), click on the "Create or edit" button to be taken to the complaint input page. Fill out the necessary fields, and click on submit once you are done. You can always return to that page by entering its title in the input box on this (Main) page, or you can search for it using the sortable table below.
